What this document looks like

A watermarked form carrying the act record number, the date of registration, both spouses’ full names, dates and places of birth, and critically the surnames assigned to each spouse after the marriage. Soviet-era versions carry the same fields on older stock.

What has to appear in the translation

A certified translation must be complete — not the important parts, all of it. For this document in particular, these are the elements most often left out by other providers, and omission is the leading cause of a Request for Evidence:

The post-marriage surname field — this is how you demonstrate that the woman on the birth certificate and the woman on the passport are the same person
The act record number and date of registration, distinct from the date of issue of the certificate itself
Both parties’ patronymics, which USCIS will compare against the birth certificates
Registry office name and district in the seal text

Send the reverse side. Corrections, re-registrations, endorsements and later stamps live on the back of the page far more often than people expect. If the back is blank, tell us it is blank — that way we both know it was checked.

Does USCIS require the translation to be notarized?

No. 8 CFR 103.2(b)(3) requires a full English translation plus the translator's signed certification of completeness, accuracy and competence. Notarization is not required by USCIS. We include it free because consulates, courts and schools abroad frequently do want it.

Can I send a photo taken with my phone?

Yes. Make sure all four corners are in the frame, nothing covers a seal, and you photograph the reverse side as well if anything is printed or stamped there.
